Bombos Medallion
Location: Fairy's Pond (Lupo Quarry, Light World)
Obtained by throwing the Mushroom into the Fairy's Pond in the Light World version of Lupo Quarry.

NOTE:
If you do not already have the Quake Medallion, this will remove the Mushroom from your inventory, allowing you to collect the second Mushroom in order to create Magic Powder, which is necessary for getting it. If you already have the Magic Powder, you must first trade it for the Quake Medallion then collect the second Mushroom without getting the Magic Powder again, or else you will not be able to pick up the second Mushroom due to a bug in the game.
Book of Mudora
Location: Village Library
Press X + R to reveal a secret door through one of the bookshelves. Dash into the bookshelf to knock the Book of Mudora to the floor.

Boomerang
Location: Guardhouse Sewers
Big Chest item. The big chest is easy to miss, it is right at the beginning of the Sewers, to the east (the main path will take you west).

If you collect the Boomerang, you will be unable to collect the Magic Boomerang later on due to a bug in the game.

Location: Misty Forest
A salesman near the entrance to the Misty Forest will sell you a bottle for 100 rupees.

Obtained by throwing the Magic Powder into the Fat Fairy's Pond in the southwest corner of the Lake Ruins.

If you do not already have the Bombos Medallion, this is the only way to remove the magic powder from your inventory in order to collect the second mushroom necessary for getting it. You must not pick up the Magic Powder from the Potion Shop until you have collected the second Mushroom and traded it for the Bombos Medallion or you will be unable to get it at all due to a bug in the game.
